    I think you're getting shielding mixed up with something else. People that were told to shield were not allowed to leave the house at all. They also had to distance from anyone in their household - the advise was not to be in the same room.

    Also the ground that was shielded is not the same group being talked about as having underlying conditions, its a tiny fraction of that much bigger group.

    But again you have to consider the percentage of the population that 'health issues' applies to. A recent NHS study puts 67% of the population as overweight. So the number of people overweight who are hospitalised, while above the national average, is not that much over the average.

    All seems a bit random. The government seems incapable of taking charge if they're telling places they should be in certain tiers but having local authorities reject it. Add in there's seemingly no threshold for going in to certain tiers and it all seems a bit like guesswork which yet again...
