Why has it become the trend to mis-pronounce "Anthony" with a TH in the middle rather than the H being silent, as intended?

I think throughout history, the evolution of names has largely been based on people's inability to spell. So many names have been mis-spelt but are phonetically correct. This happens with surnames, streetnames and place names too, and often happened by people who COULD write trying to interpret...
