They dont need pauses in game play anymore for adverts. I watch a fair few South African and Asian streams and they simply minimise the live footage to a portion of the screen and have adverts in the remaining space. Won't be long before we get that. Sky are already doing it in Australia:

There'd clearly be caveats to it. If the play is near the casualty, then ref can stop play. But if the play is moving elsewhere then wave the physios on. To often player feign injury to stop promising breaks periods of pressure/possession. Players should be stopped from going to the touchline...

I mocked this up during the summer. Before they started to sell Block 5, reduce away in Block 6 and more recently sell a few more in Block 10. I think the actual capacity is around 31500-31750
