I might be jumping the gun here but it looks like a murder/suicide, if so it's the second mother and son such incident in London in the last week or two.
Could this be a consequence of COVID and the lockdown?

Boy, 7, discovered drowned in London home with mother also found dead

The bodies of Yulia Gokcedag and her son Timur are discovered in Tower Hamlets after they were reported missing by family members.

Real heart breaking stuff.
Also, inquest in December due to backlog so a long time for the family to wait to get answers.

Edit - put on wrong forum, apologies, could someone move it to off topic.

Christ, that is awful.

Not looking for anyone else and seems possible failing somewhere, either historically or in the response. Surely there must be some record of domestic trouble/mental health the police or social services knew about, assuming she drowned her child then killed herself. You would think if lockdown was getting to you, you'd seek help before doing this.
