White card issued for first time in Portuguese football match

The card was shown during a women's cup match after medical staff from both sides rushed to help a fan who had fainted in the stands, with spectators at the match between Sporting Lisbon and Benfica cheering and applauding the decision.

So if someone does some fair play does it cancel out a yellow?

I don't get it. What exactly is the point? Every time a player makes a clean tackle and doesn't foul a player, the ref holds up a white card?

When exactly is he supposed to flourish this?

It's an absolute nonsense isn't it?
